The Triple Tax Advantage, Explained

No other account in the US tax code provides three tax benefits simultaneously:

  1. Tax-deductible contributions: Contributions reduce your taxable income — like a traditional IRA or 401(k). For W-2 employees making payroll contributions, FICA taxes (7.65%) are also avoided.
  2. Tax-free growth: Investments inside the HSA grow without capital gains or dividend taxes — like a Roth IRA.
  3. Tax-free withdrawals for medical expenses: Distributions for qualified medical expenses are completely tax-free — no income limit, no phase-out.

After age 65, HSA funds can be withdrawn for any purpose (not just medical) — they are taxed as ordinary income, same as a traditional IRA. But for medical expenses, they remain tax-free forever.

2025 Contribution Limits and HDHP Requirements

  • Self-only HDHP coverage: $4,300 annual contribution limit
  • Family HDHP coverage: $8,550 annual contribution limit
  • Age 55+ catch-up: Additional $1,000 per person (not per household)

  • HDHP out-of-pocket maximum (self-only): $8,300
  • HDHP out-of-pocket maximum (family): $16,600

You must be enrolled in an HDHP during the period you want to contribute. Medicare enrollment disqualifies HSA contributions (but you can still spend existing funds tax-free on qualified medical expenses including Medicare premiums).

The Stealth Retirement Strategy: Pay Out of Pocket, Invest the HSA

There is no deadline to reimburse yourself from an HSA for past medical expenses. If you pay $3,000 in medical bills out of pocket today and save the receipts, you can withdraw $3,000 from your HSA tax-free in 20 years — after your investments have grown significantly.

Strategy: contribute the maximum, invest in index funds, pay all medical expenses out of pocket, save receipts digitally. In retirement, the HSA becomes a tax-free reimbursement engine for decades of accumulated medical expenses.

Qualified Medical Expenses

IRC §213(d) defines qualified medical expenses. The list is broader than most people realize:

  • Doctor visits, hospital stays, surgery, dental, vision, orthodontia
  • Prescriptions and over-the-counter medications (CARES Act permanently expanded OTC coverage in 2020)
  • Mental health therapy, substance abuse treatment
  • COBRA premiums, long-term care insurance premiums (age-based limits)
  • Medicare premiums (Parts A, B, C, D) after age 65
  • Does NOT include: health insurance premiums while employed (before age 65), cosmetic procedures, gym memberships (without medical necessity letter)
